About the Role

As a Breezly Care Advocate, you'll work directly with Medicare patients living with serious or chronic conditions—such as cancer, heart disease, and chronic pain. You'll support them across the care continuum, from interpreting care plans and coordinating follow-ups to identifying social needs and ensuring services are in place. Many of the patients you'll support qualify for Principal Illness Navigation services, and your experience in this space will directly impact care quality and outcomes. This is not a general customer support role. We're looking for care professionals with deep experience in case management, care navigation, or clinical coordination, especially within Medicare or other complex care environments.

Key Responsibilities

  • Serve as a trusted partner for Medicare patients, helping them understand diagnoses, treatment options, and next steps
  • Identify barriers to care—including transportation, food access, housing, and financial strain—and coordinate resources accordingly
  • Collaborate with healthcare providers, social workers, and caregivers to ensure continuity of care
  • Navigate coverage and reimbursement, with a working knowledge of billing practices for Principal Illness Navigation (PIN)
  • Accurately and consistently document interactions and progress using Breezly's tools and protocols
  • Provide feedback on Breezly's care model to help refine and improve our services

Requirements

  • 2+ years in care navigation, case management or patient advocacy, ideally within a health system, health plan, or community-based organization
  • Familiar with the needs of Medicare patients, including how to support older adults and individuals managing chronic or serious illnesses
  • Comfortable with care coordination, patient education, and resource connection
  • A clear communicator with strong documentation habits and a compassionate, patient-first mindset
  • Comfortable working independently and remotely, while staying aligned with a collaborative, mission-driven team
  • Tech-friendly and able to learn new systems quickly

Nice to Have

  • Experience supporting patients through cancer, chronic pain, or complex, ongoing conditions
  • Familiarity with Medicare billing—especially Principal Illness Navigation (PIN) services
  • Licensed social worker, nurse, or other clinical credential (not required)
  • Multilingual or bilingual proficiency
  • Remote work experience using tools like Slack, Zoom, and EHR systems
